How do I get a Disabled parking Plate/Placard?

Submit a completed TR-159 Certification of Disability form, signed by a licensed medical professional. Follow the instructions on the form, then take or mail it to the Treasurer’s Office at 2601 W. 6th Street to apply for a disabled parking placard and/or plate. You may have up to two devices (one plate and one placard, or two placards). There is no fee for placards or the ID card.
