Where can I get a transfer on death deed form?

Douglas County Register of Deeds does not provide blank forms. However, they do provide the service of recording the prepared document for a fee. See the most up to date Register of Deeds Fee Schedule and look for these:

  • Deeds, mortgages, or other instruments — first page (max 8½" × 14", includes technology fee)
  • Second page and each additional page or fraction thereof (includes technology fee)

Suggested Steps

  1. See Article 35 - Transfer of Death at the Kansas State Legislature website.
  2. Draft a document yourself or with the help of an real estate attorney. Consider using the verbiage provided in Article 35 or search online for a Kansas Transfer on Death Deed form. A local title company may also be able to provide assistance.
  3. Get the document notarized.
  4. This all must be done before time of death.
  5. Present the prepared document to the Douglas County Register of Deeds.
